What is the Kaiser Permanente 3 to PhD® Wellness Center

The Kaiser Permanente 3 to PhD Wellness Center is a 2,500-square-foot space at the new Faubion School + Concordia University, a 3 to PhD Community building, where Kaiser Permanente clinicians will provide a full range of primary care services, including medical and dental care. They will handle a variety of health needs, from treatment of acute and chronic conditions to prevention services.

This innovative clinic is a collaboration with 6 core providers, including Kaiser Permanente, Portland Public Schools, Faubion Elementary School, Concordia University, Trillium Family Services, and Basics (formerly Pacific Foods) — all working together to create safer, healthier, and more educated communities.

Clinic features & services

When is the Wellness Center open?

Hours of operation are Monday through Friday, 7 a.m. to 4 p.m. (closed 11 a.m. to noon). The Wellness Center is also open during school closure days, such as teacher planning days and school breaks. The Wellness Center is closed during major holidays.

How do I schedule an appointment?

Appointments can be scheduled by calling the clinic during business hours, Monday through Friday, 7 a.m. to 4 p.m. at 503-916-5908. Patients can also walk in for the next available appointment or to schedule a future appointment.

Is this a free clinic?

No, the 3 to PhD Wellness Center operates like other Kaiser Permanente facilities. However, patients don’t have to be Kaiser Permanente members to seek care here.

Who is eligible for care?

We provide care for staff and faculty of Faubion School and Concordia University, Faubion students’ families, dependents of Concordia students, as well as continuing to serve students of both Concordia University and Faubion. The Wellness Center serves both Kaiser Permanente members and nonmembers.

Who qualifies as a Faubion student family member?

A family member is a mother, father, legal guardian, sibling, or foster child living in the home of the Faubion student.

Who qualifies as a Concordia student dependent?

A dependent is any child or adult that is a legal dependent of the student, this can include children or adult parents that are the student’s legal dependent.

Do you write and fill prescriptions at the 3 to PhD Wellness Center?

Clinicians can write prescriptions, and they may be filled at nearby pharmacies; members may save money by using Kaiser Permanente pharmacies or mail-order services.

Dental Coverage & Care

Who is eligible for dental care?

We provide dental care to the same patients listed above. The Wellness Center dentist serves both Kaiser Permanente members and nonmembers.

What services are available to Kaiser Permanente dental members?

All Kaiser Permanente members who have dental coverage can be seen, and the services provided will be based on their coverage. If there is a need for specialty services, we may refer them for treatment at another Kaiser Permanente dental office.

What services do you provide to Oregon Health Plan members?

We provide all covered dental services for OHP children and adults, which are:

  • Basic preventive services including exams, cleanings and sealants
  • Basic restorative services including fillings, stainless steel crowns and extractions

What services are available to patients with dental coverage that is not Kaiser Permanente?
Patients who have commercial dental coverage other than Kaiser Permanente dental can be seen, and the services provided will be based on their coverage. If the insurance carrier does not cover the treatment, the patient will be billed for the services.

Pricing & Payment

What is the cost for KP members?

Kaiser Permanente medical and dental members pay the primary care visit cost share (copay or coinsurance), based on their plan. Please refer to your Explanation of Coverage for specific benefit information.

What is the cost for nonmembers?

Costs may vary based on service(s) provided. Patients will not be denied care, nor will care be deferred based on the ability to pay. If a patient has difficulty in paying, they may be eligible for financial assistance.

Do you accept insurance plans other than Kaiser Permanente?

Patients may be asked for a payment* at the time of service. For patients with insurance providers other than Kaiser Permanente, we will bill the insurance carrier. The patient will then be billed for any balance that their insurance provider does not pay.

*Unaccompanied minors will not be asked for payment at the time of service.

What forms of payment do you accept?

If payment is made at the time of service, we accept any major credit or debit card, including Health Savings Account (HSA) cards. We are not able to accept checks, cash, or money orders.

What if a patient doesn’t have insurance or can’t pay?

Patients may receive care at the Wellness Center regardless of their insurance status or ability to pay. If insurance does not cover the services and he patient is unable to pay for the bill, we will connect them with patient financial services for support. If needed, this counselor can screen families for available programs including Medicaid and Medicare as well as identify other appropriate resources.
